Is there a confidentiality/privacy issue?
Are there any vacations, plans, etc., of which you are aware, which would interrupt your participation in Group Therapy/Support during the next 26-28 weeks?
A brief statement on the pattern you wish to have help with.
Tell me how you will prefer to pay: (One-time payment of $997.00 in advance (you get the Extra Bonus half-hour with Jannette); or three payments over the first 8 weeks (of the 26-week group) = $332.34 at Registration, $332.33 at 4th week, and at 8th week.)
Your preferred payment method: (Cashier’s check by postal mail; PayPal (where available — you can use your credit card with PayPal, via either the single or triple payments); wire cabled funds, or if none of these are available to you, give me your proposed special payment arrangements.

SEND NO MONEY NOW. PAYMENT ARRANGEMENTS WILL BE AVAILABLE AND YOUR PAYMENT REQUESTED AT THE TIME YOU ARE APPROVED FOR PARTICIPATION, AND YOUR GROUP IS SCHEDULED.
